Services are based on an hourly rate. CNS does not participate with insurance, which means we are "out-of-network." As such, it is your responsibility to pay for the evaluation in full.
Depending upon your coverage, your insurance carrier may provide partial or full reimbursement to you for our services. It is however, your responsibility to contact your insurance company to discuss your Mental or Behavioral Health benefits should you be interested in submitting a claim. We will provide you with the necessary information to do this; however, we cannot guarantee if they will provide you reimbursement. The final fee for an evaluation depends on several factors, including the presenting concerns and level of additional contacts needed (e.g. a school observation, attendance at a Planning and Placement Team meeting, etc.). We are happy to provide additional information about cost when you call. Payment in full, in the form of a retainer, is expected at the time of your first appointment unless alternative plans have been agreed to. Should the final cost of the evaluation differ from our original estimate, we will either reimburse you the difference in the form of a check or send you a final invoice, to be paid prior to releasing the written report. We accept cash, checks, and major credit cards. |
Specifically related to the assessment, your insurance company will require the following CPT Codes to determine what level of coverage you may have: Psychological Assessment 96101 Neuropsychological Assessment 96118 Comprehensive Intake (1 hour) 90791 |
